We understand how important the portal is for managing your healthcare, and we are working diligently to make this transition as smooth as possible.
Our new patient portal, Follow My Health, will be available to you sometime in October and more details – including information about the new portal features & the exact launch date – will be shared with you in the coming weeks.

A: No information will be stored in the patient portal and no users will have access after 9/30/25.
Q: Can I download or save the info that’s in my existing patient portal?
A: Yes, you can download documents, visit summaries, and clinical notes.
Q: What happens if I didn’t download or save the information from my previous patient portal account?
A: If you did not download the information prior to 9/30/25, you will no longer be able to access your info.
