Depending on which urology provider you are transferring from, we might even have the necessary form for you to fill out.
We accept most Insurance plans. Please contact us at (866)367-8768 to review our participation with your particular insurance payer.
Please contact the main number and speak to the Physician on call with any urological emergencies. The physician on call will help direct you to the appropriate emergency facility.
It’s always your choice to select your medical provider! For continuity of care, we recommend you schedule your appointment with your current Urology Partner physician who will be happy to provide you with a consult to determine the best Urological Specialist for your needs.
Yes, please call for an appointment. You must see a Urology Partner Provider at some point before starting or continuing your injections if transferring your care.
